Dietary advice for IBD is one of the most confusing areas patients navigate. Conflicting recommendations, social media claims, and anecdote-driven protocols make it hard to know what's actually supported by evidence. This page reviews the clinical trial data for the most commonly discussed dietary approaches — with honest assessments of what the evidence shows, what's uncertain, and what matters most for Crohn's disease and ulcerative colitis.
IBD is an immune-mediated disease — but diet interacts with IBD through multiple mechanisms: gut microbiome composition, intestinal permeability, mucosal immune activation, and symptom generation. The challenge is that these mechanisms are distinct. A diet might reduce symptoms without reducing inflammation. A diet might reduce inflammation in animal models without doing so in humans with established IBD.
Most dietary trials in IBD are also small, short-term, unblinded, and highly heterogeneous in patient populations. This limits the strength of conclusions. Where strong evidence exists, it's noted. Where it doesn't, that matters too.
Important distinction: Symptom improvement and mucosal healing are not the same thing. Several diets reduce IBD symptoms (bloating, pain, frequency) through mechanisms that don't affect underlying inflammation. For patients aiming to achieve or maintain mucosal remission, dietary trials showing only symptom endpoints may not be sufficient evidence.
Exclusive enteral nutrition (EEN) — replacing all food with a liquid formula for 6–8 weeks — has the strongest dietary evidence base in IBD. In pediatric CD, EEN achieves remission rates comparable to corticosteroids (around 60–80%) while also promoting mucosal healing and growth, which steroids do not. It is recommended as first-line induction therapy for pediatric CD by ECCO/ESPGHAN guidelines.
In adult CD, EEN achieves remission in 50–70% of patients but is less consistently used due to adherence difficulty. It's particularly effective for small bowel disease and as a bridge therapy during biologic optimization.
Key data: A 2019 Cochrane review (12 trials, 1049 participants) found EEN equivalent to steroids for inducing remission in CD (RR 0.97, 95% CI 0.78–1.20). Post-treatment relapse rates are similar but EEN improves nutritional status and growth in pediatric patients.
The Specific Carbohydrate Diet eliminates most grains, processed sugars, and many dairy products on the hypothesis that specific carbohydrates feed pathogenic bacteria. It has strong patient-community support and many reported anecdotal successes.
The most rigorous trial data comes from the PRODUCE study (2022, NEJM) — a pediatric CD study comparing SCD to a Mediterranean diet. Both diets showed similar rates of clinical response and remission, with no significant difference between them. Neither was compared to standard-of-care pharmacotherapy.
PRODUCE trial (NEJM 2022): Among 194 children with mild-to-moderate CD, SCD and Mediterranean diet achieved comparable 12-week remission rates (~46% vs. ~45%). No placebo arm. Mucosal healing not assessed as primary endpoint. Notable: NEJM publication signals legitimacy, but the trial design limits conclusions about mechanism.
Whether SCD reduces mucosal inflammation (not just symptoms) in adults has not been established in adequately powered trials. The elimination components make it difficult to identify the active mechanism. For adults with moderate-to-severe disease, SCD is not evidence-supported as a replacement for pharmacotherapy.
The low-FODMAP diet (low fermentable oligosaccharides, disaccharides, monosaccharides, and polyols) was developed for IBS and has strong evidence for functional bowel symptoms. Its application to IBD is important to understand clearly: it is effective at reducing symptoms in IBD patients, but there is no convincing evidence it reduces mucosal inflammation.
Many IBD patients also have concurrent IBS-like symptoms — one study found 33–57% of IBD patients in remission still have IBS-type symptoms. Low-FODMAP can be valuable for this population but should not be confused with anti-inflammatory treatment.
Clinical utility: A 2016 randomized controlled trial (Pedersen et al.) found 52% of IBD patients on low-FODMAP reported symptom reduction vs. 16% control. However, fecal calprotectin (inflammatory marker) did not decrease, confirming symptom benefit is functional rather than anti-inflammatory.
Long-term low-FODMAP can reduce prebiotic fiber intake and alter gut microbiome diversity. It should be implemented with a dietitian experienced in both low-FODMAP and IBD, and not maintained indefinitely without structured reintroduction.

The diet's anti-inflammatory profile (omega-3 fatty acids, polyphenols, fiber, reduced processed food) aligns mechanistically with IBD pathophysiology.
The PRODUCE trial placed Mediterranean diet on equal footing with SCD in pediatric CD for clinical response. Adult IBD trials are ongoing. The Mediterranean diet is generally well-tolerated, nutritionally complete, and sustainable — making it a reasonable general dietary framework even where IBD-specific trial data is limited.
Epidemiology vs. trials: The PREDIMED study and others show Mediterranean diet association with lower C-reactive protein and inflammatory markers in general populations. Direct IBD RCTs in adults are limited. But the risk profile is low and nutritional adequacy is high — which distinguishes it from more restrictive approaches.
The Crohn's Disease Exclusion Diet (CDED) was developed to eliminate dietary components hypothesized to impair gut barrier function — specifically processed foods, emulsifiers, gluten, animal fat, and certain proteins. It is typically combined with partial enteral nutrition (PEN).
The CDED+PEN combination showed strong results in a 2019 RCT (Sigall-Boneh et al., Gastroenterology): 75% clinical remission and 57% fecal calprotectin normalization at week 24 in pediatric CD, compared to EEN which achieved higher early remission but worse sustained response.
Why this matters: CDED+PEN achieves mucosal outcomes (calprotectin normalization), not just symptom improvement. It's the most compelling dietary intervention for anti-inflammatory effect beyond EEN. Adult trials are underway — the DIET-CD trial in adults is expected to report.
Regardless of dietary approach, nutritional monitoring is a clinical priority in IBD. Malabsorption, food avoidance due to symptoms, and systemic inflammation all contribute to deficiency risk:

A note on restriction: Highly restrictive diets carry real risks in IBD — malnutrition, micronutrient deficiency, and exacerbating the psychological burden of a chronic illness. The absence of evidence for harm isn't evidence of safety when nutritional adequacy is compromised.
